The Passenger declares that:

• His/her health condition and current physical condition are very good and he/she does not suffer from any illness or ailment that could result in a sudden deterioration of his/her health condition or of her health complications during or as a result of the tandem jump. The passenger declares that he/she does not suffer in particular from any illness that requires long-term care or regular medical attention of a medical professional (such as heart diseases, epilepsy, circulatory system diseases, etc.);
• In the period of 24 hours immediately preceding the tandem jump, he/she has not consumed any alcohol, narcotic or psychotropic substance, or any similar substance;
• He/she has decided to perform the tandem jump voluntarily and of his/her own free will and has carefully considered his/her health condition before the jump;

Operator safety training according to SPO OM-D and tandem jump instructional
training:

• The client has been acquainted with and will follow the tandem skydiving instructional training and the SPO operator safety training regarding movement at the dropzone and the flight by the airborne aircraft.
• The client is expressly prohibited from taking any foreign objects on the airborne aircraft, especially valuables such as chains, earrings, rings, bracelets, and watches, as well as mobile phones or other recording devices (cameras).
• If you wear glasses, we recommend that you take contact lenses for the tandem jump. Unless otherwise stated, you can jump wearing glasses, but at your own risk – we are not responsible for their damage or loss.
• It is strictly forbidden to grab or hold your tandem master’s arms while in free fall! Such actions could cause a disruption of stability during your skydive.
• Move only in the designated areas of the dropzone and strictly follow the instructions you received during your training and from the dropzone’s staff.
• If you have any questions, do not hesitate to ask your tandem master or the dropzone’s staff. In the event of an emergency, follow the instructions of your tandem master or dropzone’s personnel.
